The Skinny on Tirzepatide & Semaglutide:

What is Medical Weight Loss?

Medical weight loss, or medically supervised weight loss, is a specialized weight loss program designed and overseen by healthcare providers. It typically involves a combination of dietary changes, physical activity, and medication.

  • Step 1: Our clinicians analyze every aspect of a patient’s health, including their medical history, symptoms and exercise/nutritional habits.  (*while we do not require blood work for these consultations, we can discuss the need for ordering this and trending your metabolic markers if interested!)
  • Step 2: While not mandatory, it’s highly recommended each patient work with our Registered Dietitian to create an individualized plan with dietary recommendations and exercise suggestions to promote healthy lifestyle options for better, more sustainable results.

Which Interventions Help with Weight Loss?

Losing weight involves leading a healthy lifestyle, including eating healthy and working out regularly. However, sometimes these methods are not enough, and that’s where medications come in to give that extra push. Medications can be an effective aid in the battle against unwanted weight gain and chronic inflammation that can reduce longevity.  They give you an added boost to help shed those extra pounds and increase your chances of success, especially if you’ve hit a weight-loss plateau.  Here at Advanced Vitality HRT, we are pleased to offer the following options to help you live a happier, healthier and longer life:

  • Tirzepatide: A ‘first-in-class’ medication, this innovative medical option works by lowering glycemic levels, improving insulin sensitivity, reducing body weight by over 20% and enhancing lipid metabolism. Tirzepatide is a dual-receptor agonist that targets two hormones, GLP-1 and GIP, which play critical roles in regulating glucose and blood sugar levels. Tirzepatide’s success in achieving medical weight loss comes from its ability to suppress appetite and regulate blood sugar levels. It works by targeting two specific receptors in the body, which triggers the body to produce insulin and glucagon-like peptide 1 (GLP-1). GLP-1 hormones work by slowing down digestion and making people feel full for longer, leading to an overall reduction in calorie intake. Studies have shown that Tirzepatide can significantly improve weight loss. Additionally, the medication has been shown to improve other aspects of metabolic health, such as controlling blood sugar and reducing inflammation.

  • Semaglutide: Works by mimicking the effects of a hormone in the body that regulates blood sugar levels and appetite. This means that when Semaglutide is taken, it can help reduce feelings of hunger, leading to less food intake and weight loss. It has been shown to be particularly helpful for those who have struggled with losing weight through other methods, as well as those who suffer from health issues associated with weight gain and inflammation, such as diabetes and heart disease. Semaglutide works by targeting the GLP-1 receptors in the pancreas, brain and stomach. When these receptors are stimulated, they reduce appetite and slow down digestion. As a result, individuals snack and overeat less, which ultimately contributes to weight loss. Delayed digestion can also have additional benefits, such as improved blood sugar control, which is essential in managing diabetes. Semaglutide has also shown potential in reducing the risk of several health complications including heart disease, stroke, and even certain types of cancer. By promoting weight loss, Semaglutide can lead to better health outcomes and reduce the burden of chronic disease.

  • Peptides: Peptides are small chains of amino acids found naturally in the body. They play an essential role in various bodily functions, including growth and repair. When used in combination with a healthy diet and exercise program, peptide therapy can aid in fat loss, muscle growth and metabolism regulation. Peptides work by stimulating the release of human growth hormone, which helps speed up weight loss and improve body composition. Additionally, peptide therapy has been found to increase energy levels and reduce appetite, making it easier for patients to stick to a healthy lifestyle.  Advanced Vitality HRT is pleased to offer multiple peptides.

  • Nutritional consulting: This form of consulting primarily focuses on creating a personalized nutrition plan that caters to the individual’s body type, medical history and lifestyle. A registered dietitian crafts a specialized meal plan that will help them achieve their weight loss goals while also ensuring they receive optimal nutrition. Nutritional consulting provides a holistic approach to weight loss that considers all facets of a patient’s health, making it an effective method for anyone looking to shed pounds in a safe and sustainable way.

How do side effects compare between Tirzepatide and Semaglutide?

Side effects are similar between tirzepatide and semaglutide, because they work in a similar manner. Stomach side effects like nausea, vomiting, and diarrhea are most common for both tirzepatide and semaglutide and can occur in over 50% of all patients.

Stomach side effects are usually transient and decrease over the first few months of treatment. Roughly 4% to 8% of patients in studies stopped treatment, usually due to stomach side effects like nausea, vomiting, and/or diarrhea.

Most reports of nausea, vomiting, and diarrhea occur when doses are being increased at the start of treatment. Here at Advanced Vitality HRT, our clinicians titrate the dose depending on your individual response, which may help to reduce stomach side effects with tirzepatide or semaglutide.  We can also prescribe anti nausea medication which can help while your body is adjusting.  Our registered dietitian can help guide food choices which can alleviate many gastrointestinal side effects.

These agents do not usually cause low blood sugar (hypoglycemia) unless they are combined with other medicines that may cause hypoglycemia like the sulfonylureas or insulin.  It is important you let your clinician know all your current medications.

Serious Warnings

The labeling for both tirzepatide and semaglutide carry Boxed Warnings, the FDA’s strongest safety warning, for possible an increased risk of thyroid tumors, including cancer, which has been seen in animal studies.

Do not use these products if:

  • you or your family has a history of medullary thyroid carcinoma (MTC), or
  • if you have an endocrine system condition called Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

Tell your provider if you have a history of pancreatitis, diabetic retinopathy (vision changes), hypoglycemia (low blood sugar), kidney problems, gallbladder disease, or an allergy to any active ingredients in the tirzepatide or semaglutide product.

Both tirzepatide and semaglutide can slow gastric emptying (which is slowed or stopped movement of food through your digestive tract). Tell your healthcare provider if you have a history of gastroparesis (slowed stomach motility).

In 2023, the FDA updated the side effects for tirzepatide and semaglutide to emphasize that an ileus, an intestinal blockage, has been reported as a possible side effect. The FDA cannot reliably determine how often this side effect occurs or if the drug actually caused the ileus due to the voluntary nature of the reports. Speak to your clinician about your risk for this side effect.

Also, tell your healthcare providers if you are scheduled for any procedures or surgeries where you may receive any type of anesthesia.  It’s important to note that tirzepatide/semaglutide can slow down digestion, which will delay your stomach from emptying and an increased risk of complications under anesthesia. If you are taking tirzepatide/semaglutide, speak with your anesthesiologist or surgeon before undergoing any procedure that requires the use of anesthesia.

While you are taking these medicines, pay close attention to any changes in your mood, behaviors, feelings, or thoughts and call your healthcare provider right away if you have any changes to your mental health, feelings of depression or thoughts of suicide.

Semaglutide and tirzepatide may decrease the absorption of other oral medicines due to the delayed gastric-emptying effect. Check with your healthcare provider or pharmacist if you take other oral medicines.

The manufacturer of tirzepatide advises females using birth control pills to switch to a non-oral contraceptive method or add a barrier method of contraception (like a condom) for 4 weeks after you start treatment and for 4 weeks after each dose increase.

Potential Pitfalls of Tirzepatide and Semaglutide

LOSS OF LEAN BODY MASS

As weight drops significantly, it’s common to see a decrease in both lean mass (muscle and bone) and fat mass. Individuals using Tirzepatide/Semaglutide may experience some loss of lean muscle mass. However, our clinicians are equipped to explore strategies to minimize this loss. It’s crucial to team up with our registered dietitian to develop a comprehensive plan, incorporating dietary guidance and resistance training. This sets the stage for optimal outcomes. Now, let’s tackle the next potential challenge…

LOWER BASAL METABOLIC RATE

With significant weight loss often comes the assumption of some reduction in lean body mass, leading us to pitfall number two. When individuals stop using these medications, their basal metabolic rate decreases, resulting in fewer calories burned at rest. Despite being at a lower weight, their percentage of body fat and muscle likely declined. As a result, they may experience a resurgence of appetite and food intake, coupled with fewer calories burned at rest, leading to weight regain. Sometimes, this weight regain can surpass previous fat mass levels, making future weight loss even more challenging.

However, there are several antidotes to this pitfall. One option is for Tirzepatide/Semaglutide users to consider staying on the medication at a low dose for weight loss maintenance.

Additionally, focusing on a high-quality diet with sufficient protein intake while using these medications is essential. A nutrient-rich, protein-rich diet supports lean muscle mass and aids in weight maintenance.

Incorporating strength/resistance training into your routine can also be highly beneficial. This type of exercise helps counteract the reduction in muscle mass associated with significant weight loss and prevents a drastic decrease in basal metabolic rate. Remember, the more muscle you have, the more calories you burn at rest.

This is why we strongly advise working with our registered dietitian, especially at the beginning, to establish a solid foundation for nutrition and weight training. Together, we can ensure the best possible results.

SLOWED GUT MOTILITY

The speed at which food travels through our digestive system is crucial for various bodily functions, including digestion, absorption of nutrients, blood sugar regulation, and waste elimination. Tirzepatide/semaglutide works by slowing down gut motility, providing a beneficial effect during mealtime by allowing ample time for the breakdown and absorption of nutrients in the small intestine.

However, this slowed gut motility can sometimes lead to issues like constipation, akin to a trash buildup in our system. Regular elimination is vital for removing toxins from both internal biochemical waste and external exposures, reducing the risk of disease. Therefore, it’s essential to support your body’s elimination processes while on tirzepaitde/semaglutide.

Emphasizing a nutrient-rich diet, particularly rich in vegetables and plant fiber, is crucial. In some cases, supplementation may be necessary to maintain normal elimination. Although the impact of these medications on gut microbiota is not fully understood, GLP-1 may potentially reduce gastrointestinal inflammation and improve gut lining barrier function. However, slowed gut motility poses a risk for small intestinal bacterial overgrowth, which can cause symptoms like bloating and changes in stool quality.

While data on the exact effects of these medications on gut microbiota remains limited, adopting mindful eating habits such as chewing food slowly and supporting the production of digestive enzymes can aid in smooth digestion and help maintain microbial balance. So, chew your food well, ditch the multitasking, and savor each bite mindfully while using tirzepatide/semaglutide for optimal digestive health.

BOOMERANG HUNGER

The most critical challenge lies in averting the rebound hunger that often occurs when abruptly discontinuing these medications, especially after reaching higher doses.

For those utilizing these medications for short-term weight loss objectives, I recommend a gradual tapering-off approach. This allows your body’s natural balance mechanism, known as homeostasis, to adjust smoothly. Even when these medications are prescribed within appropriate clinical contexts, it’s essential to implement a comprehensive lifestyle plan when tapering off. It’s crucial to understand that they are intended for long-term treatment from the outset.

Never discontinue tirzepatide/semaglutide abruptly without the guidance and support of your clinician, alongside a structured lifestyle management plan in place. Smooth transitions ensure the best outcomes for your health journey.

Here at Advanced Vitality HRT, we encourage the use of medically supervised weight loss to assist our patient’s in reaching their goals, as well as improve inflammatory markers and internal health.  We understand there is a lot of chatter in the news and social media regarding tizepatide/semaglutide, often times portraying these incredible medications in a negative way.  We firmly believe the positive effects of these medications in not only enhancing a patient’s self confidence and body image but also their effects on lowering inflammation, maintaining a healthy body weight and improved internal health markers.

I'M READY TO LOSE FAT!

Although there’s no prescribed diet for tirzepatide/semaglutide success, our registered dietitian Alyssa Cooke RD is here to assist you in fine-tuning your nutrition and resistance training plan. With her guidance, you can achieve optimal results while minimizing potential side effects, ensuring the best possible outcome all while living a healthier lifestyle! Mariah went to Rock Valley College in Rockford, Illinois to graduate with her degree in Respiratory Therapy in 2019. She has worked as a respiratory therapist on night shift since 2018, working for Mercy Health in Rockford, Illinois for her whole career thus far. During her career she worked all over the hospital in all departments including the emergency room, pediatrics, Adult Critical Care, and the Neonatal Intensive Care Unit. As of 2022 she also became part of the Neonatal Transport Team.

While she loves her job as a respiratory therapist, she decided that even though she loves helping people, and being part of the team to heal patients; the hospital atmosphere is not where she wants to be every day. Becoming part of the Advanced Vitality team is something she is very passionate about because she is able to still help people achieve their personal health goals, all while having a more personable business to work for rather than a large hospital.
Mariah currently lives in Rockton, Illinois on a farm with her husband and two children.

She tries to spend a lot of time outside caring for the animals, riding their horses, and just exploring their land with her kids. Most days are spent at home, or doing something new outside somewhere when the weather is nice. That is why she feels Advanced Vitality was a great fit for her because it allows her more time with her family and allows for a more normal schedule all while doing what she loves- helping others.

Since 2015, Troy has successfully completed multiple investments in tech start-ups operating within the multifamily housing space, purchased or invested in over 600k square feet of commercial office/industrial space, and has passively invested in over 2,500 apartment units across the Southeast and Texas markets before taking a more active role in acquiring and repositioning multifamily properties with his company SW Capital.

Jason M. Theobald graduated law school in 2004, he worked as general counsel to a company local to Cincinnati for 15 years. During this time Jason honed his knowledge of business and creating businesses. In 2017 Jason left his legal job and devoted his full time to his Nutrition Consulting Company called ScoobyHealth. He has grown it to a 7 figure business with 13 coaches.

Jason’s next venture was launching NuEthix Formulations a supplement company found on principles of honesty and integrity. He recognized what the industry was missing, the idea that what is on the label is in the bottle, and thus NuEthix was born.

Jason has 25 years experience working with athletes and weekend warriors who want to OPTIMIZE their bodies. Jason has read thousands of labs and consulted with many regarding their hormonal health. Jason has a Nutrition Consultant license from the AFPA and has studied under Dr. Charles Sefcik a functional medicine practitioner. Due to Jason’s vast experience working with those who have hormonal deficiencies, opening a TRT clinic was a natural progression for him. Jason has been quoted saying, “When you see thousands of labs of those on HRT, you start to notice patterns, and see where mistakes are being made in the administration of these meds. I wanted to bring my experience to the table so people can experience the true benefits HRT has to offer both men and women.” Jason is also 47 years old and uses HRT therapies in his daily life.

Over 10 years experience in medicine, started my career in Women’s Health, Neonatal ICU & as a Lactation counselor. Transitioned to Aesthetics & Concierge infusions, then Functional Medicine/BHRT

Why you chose functional medicine:
I found functional medicine through my own health journey. When traditional medicine had no answers for my chronic pain, dysautonomia, and gut issues, I found a functional medicine provider who spent the time to get to the root cause and help me completely change my life. This journey inspired me to learn as much as I could about this approach to medicine & pursue a career that more closely aligns with my values.

Connecting my patients with their inner wisdom and ability to heal themselves.

Personal wellness routine:
AM Routine: Meditation/Breathwork, Morning sunlight, Qi Gong, Lymphatic self massage & Skincare
PM Routine: Sauna, Red Light therapy, breathwork, facial cupping/gua sha, skincare

I love hiking, hot pilates, aerial yoga, dancing, paddleboarding/anything on the water, and traveling the world.

Memorable patient wins:
I had a patient who had been dismissed by multiple physicians who was experiencing extreme histamine response to menopausal shifts (pain, skin changes, major sleep and mood disturbances) and was in a deep depressive state as a result of CPTSD. Through a multifactorial approach including nervous system regulation, lifestyle support, supplements, and slowly titrated BHRT, we were able to eliminate her pain, get her sleeping regularly & stabilize her mood. Soon after, other areas of her life began to open doors, and she was able to shift her career to something more aligned with her new state of being.

Loraine Dragwo

Nurse Practitioner

Education and Background:
Chamberlain University – MSN for Family Nurse Practitioner

Why you chose functional medicine:
I found functional medicine through my own health concerns of GI symptoms, adrenal fatigue and early menopause. It took me years for a provider to really listen to me and figure out what was going on. It was that moment that I knew I wanted to get into functional medicine and hopefully prevent another patient from going through the frustration that I went through.

Favorite part of the job:
Picking just one is tough! I enjoy getting to know my patients and hearing their stories; figuring out the pieces to the puzzle to help patients optimize their health so that they have the energy to do the things they enjoy; and that I’m constantly learning.

Garrett Peed

Nurse Practitioner

Education/Background
Earned a Master of Science in Nursing and Family Nurse Practitioner degree from the
University of Southern California.
Spent 8 years at Johns Hopkins Hospital working as a Nurse in the Cardiovascular
Surgical Intensive Care Unit.

nutrition, and integrative pain management using platelet-rich plasma (PRP) and stem-
cell–based treatments.
Certified through the American Academy of Anti-Aging Medicine (A4M) and the Institute
for Functional Medicine (IFM).

Why I Chose Functional Medicine
Drawn to root-cause medicine after seeing gaps in traditional care, especially around
patient education and lifestyle change.
Functional medicine allows for deeper connection and understanding of the “why”
behind illness.
Provides the time and structure to help patients achieve optimal health through
education, nutrition, and lifestyle alignment.

Favorite Part of the Job
Listening to patients’ stories and understanding their health goals.
Watching small, consistent changes compound over time helping patients feel more
energetic, youthful, and empowered.

Personal Wellness Routine
Begins each day with a morning workout and a few minutes of sun exposure while
enjoying coffee.
Prioritizes quality time with his children each morning before the work day begins.
Practices mindfulness and meditation for 7–10 minutes daily.
Takes ice baths 3–5 times per week and maintains a consistent evening sleep routine.
Supplements include magnesium, creatine, and omega-3s.
Favorite peptides: NAD+ and glutathione; uses methylene blue for cognitive support.

Philosophy in One Line
“Optimal health comes from consistent daily habits, but I live by the 80/20 rule: stay
disciplined 80% of the time and enjoy life the other 20%.”

Outside the Office
Enjoys the outdoors with his wife and two children, long walks, beach trips, and
backyard adventures.
Passionate about exercise, cooking nutritious meals with family, and gardening.

Memorable Patient Win
Helped a 50-year-old woman regain her vitality after years of fatigue and medication
dependence.
Through hormone balancing, stress management, and lifestyle optimization, she
regained her energy and confidence.
Her gratitude: “Thank you for giving me my life back”, remains one of my most
meaningful professional moments.

APRN, Nurse Practitioner

Education and Background:
Endocrinology specialty certificate from Duke University
MSN Family Nurse Practitioner from University of Alabama at Birmingham
BSN University of Miami
BA Anthropology Northern Illinois University

Why you chose functional medicine:
I chose to work in functional medicine because it allows me to identify and address the root causes of illness rather than just treating symptoms. This approach lets me combine evidence-based medicine with personalized care to help patients achieve lasting health and vitality.

Favorite part of the job:
My favorite part of working at AV is empowering my patients to improve their lives and helping them to reach their goals.

Personal wellness routine:
My personal wellness routine depends on what my current goals are at the time- right now while pregnant I focus on keeping my lifestyle as intentionally focused on health as possible with weight training, daily steps, and a Lily Nichols’ inspired diet. When not pregnant, I add in testosterone replacement and peptides to support my goals of improving body composition and focusing on longevity/anti-aging!

Philosophy in one line:
“I’ve missed more than 9,000 shots in my career. I’ve lost almost 300 games. Twenty-six times, I’ve been trusted to take the game-winning shot and missed. I’ve failed over and over and over again in my life. And that is why I succeed.” – Michael Jordan

Outside the office, enjoys:
Outside of the office, you can find me working out at the gym, baking sourdough, or spending time with my husband and daughter.

Memorable patient wins:
Some of my most memorable patient wins include supporting women on their journey to conceive and helping women regain healthy menstrual cycles after years of imbalance. I’ve also had the privilege of helping postmenopausal women feel like themselves again through thoughtful hormone optimization, and guiding patients toward sustainable weight loss and lasting lifestyle changes. One particularly rewarding case involved identifying a pituitary adenoma that had gone undiagnosed by endocrinology for years—finally giving her answers after a long journey of feeling unheard.

Jeanie Taggart

APRN, Nurse Practitioner

Education and Background:
I am Board Certified Family Nurse Practitioner, graduated from the University of Cincinnati. My background and training is in functional endocrinology.

Why you chose functional medicine:
I was a nurse for 8 years, worked as a travel nurse for 3 and traveled all over the country caring for patients and the #1 questions I was always wanting to figure out is why is our health care model reactive and not proactive? When I completed my Masters degree my mission has always been to help individuals reverse and prevent disease. Functional medicine allows me to do exactly that.

Favorite part of the job:
Changing the quality of someone’s life and helping them walk through the hardest seasons of their life with a plan to come out better than they started

Personal wellness routine:
My mornings start best with prayer/quiet time and lots of sunshine when the midwest weather allows for it. I am a former athlete that now has a passion for strength training and a little Lagree.

Your philosophy in one line:
“If God didn’t make it, don’t eat it”

Outside the office:
My family is the most important thing to me. I love to be studying/reading, cooking, boating or traveling. My niece and nephew are my favorite humans on the planet so more than likely you can find me running around with one of them when I have free time.

Memorable patient wins:
Honestly, I simply love what I do… theres so many patient wins. But the most rewarding thing to hear is a patient report that they feel “heard” for the first time in their health journey.

Kennedy Salerno

Physician Assistant

Education and Background
Masters of Science in Physician Assistant Studies, Medical University of South Carolina
Bachelors of Science in Human Biology, University of Albany
Post Grad – Continuing Education:
Completion of Worldlink Medical BHRT Certification Course
Attendance and Completion of: A4M Empowering the Menopause Journey Conference, 10/2025
Attendance and Completion of: A4M Longevity Fest,12/2025

Why you chose functional medicine:
I have a strong passion for addressing the root cause of illness, focusing on treating the entire body as one system, and not treating symptoms with a “band aid”. I prefer to prioritize healing from the inside, out with my main focus in optimizing lifestyle and adding in supplements, medications and peptides where needed.

Favorite part of the job:
Being able to provide solutions that are specific to each individual patient case and witnessing the transformation when patients regain confidence, health and vitality.

Personal wellness routine:
Daily non-negotiables:
Drinking a full 1L bottle of water and taking my dogs for a walk right when I wake up in the morning
Reading/Journaling before I go to sleep at night
Favorite Peptide: NAD+, every other day.
This has helped me regain energy, focus and productivity throughout my day.

Philosophy in one line:
Self discipline is the foundation of everything you desire; how you speak to yourself and the standards you hold yourself to, directly shape the trajectory of your life. Lasting
change comes from consistent daily choices and showing up, even on the days you don’t feel like it. You can apply this to your health, career, relationships and more.

Outside the office, enjoys:
Strength training (bodybuilding and powerlifting)
Spending time with my family and german shepherd pups
Being in nature – specifically the beach when the UV is 10 (with SPF of course!)

Memorable patient wins:
Assisting a patient with Hashimoto’s in reclaiming her energy, balancing hormones, and achieving sustainable weight loss; helping her feel like she “got her life back.”
